CBT in Santa Monica and Across California

CBT has the best evidence base and the worst reputation, and both are deserved. It works, reliably, on certain problems. It is also the thing people mean when they say therapy felt like homework assigned by someone who was not listening.

I use CBT where it earns its place. When a thought pattern is doing measurable damage — you are avoiding something that is shrinking your life, or running a prediction so consistently that you have stopped testing it — we work on it directly and practically, and we do it fast. What I do not do is open a protocol on week one and work through it page by page regardless of what you walked in with.

What It Looks Like

Direct work on a thought that is costing you something

The first move is almost always specificity. Not 'I have low self-esteem' but the sentence that actually ran through your head at 4:40 on Tuesday when your manager scheduled a meeting with no agenda. People are startled by how precise their own predictions are once they slow down enough to catch one.

Then we test it. Not by arguing with it — arguing with your own thoughts is a losing format, and you are better at it than I am — but by finding out what happens. You send the email without the three softening clauses. You let the meeting be unexplained for an afternoon. We find out what the evidence does to the prediction.

Behavioral work carries more of the load than the thinking work in most cases, which surprises people who expect CBT to be about logic. Avoidance is the engine of most anxiety, and the only thing that reliably weakens it is doing the avoided thing in small, graded, deliberately unimpressive increments.

With depression, the order tends to reverse from what feels intuitive. Action comes before motivation rather than after it. Waiting to feel like it is the trap, and the first month is often mostly about scheduling things you do not want to do and noticing, honestly, what they did to the following afternoon.

Where CBT does its best work

These are the presentations where I reach for it early rather than late.

Anxiety that runs on avoidance

Panic, health worry, social dread, the meeting you keep not scheduling. Graded exposure is unglamorous and effective, and it works better when we also know what the avoidance is protecting.

Depression that has stalled activity

Behavioral activation, in small increments, against your own accurate objection that it feels pointless. We track what actually happened rather than what you expected to happen.

A single loud prediction

One belief that is running your decisions: they will leave, I will be found out, this will collapse. Worth examining directly, with evidence, before we go anywhere near its origins.

The Honest Caveat

A clean thought record is not the same as change

The failure mode of CBT is a very articulate client. Someone who can identify catastrophizing in their own sentences, complete the worksheet neatly, and go home and do exactly what they did before. The technique was fine. It was simply not addressing what kept the pattern in place.

That is why CBT here is a tool inside a broader relationship rather than the whole of the work. If we examine a prediction and it does not move, that is interesting information, and it usually means the prediction is doing a job for you that we have not understood yet.

You will also always know what I think and why. I do not run this as a neutral technician administering a protocol, and if something I suggest lands badly, telling me is useful data rather than a problem.

Honest Limits

What CBT is less suited to

Long-standing attachment and trauma patterns generally do not resolve through thought records, and I would rather say that at the start than imply otherwise. Those need slower, relational work, with skills brought in alongside rather than instead.

CBT also asks something of you between sessions. Not a binder of homework, but a willingness to try the small experiment we agreed on and report back honestly when you did not. Not having done it is workable material. Saying you did it when you did not is the only version that wastes the hour.
