When you send a consultation request, the form asks for your first and last name, your email address, an optional phone number, and the days and times that suit you.
When you book through the calendar, it asks for your name, email address and phone number, and records the time you choose.
The website does not ask for health information. Please do not include any in these forms. What brings you in is for the consultation itself.
How It Is Used
Your information is used only to reply to you and to schedule and confirm your consultation. It is not sold or rented to anyone.
The request form and the booking calendar run on service providers that deliver your request or booking to Dr. Payne and send confirmation emails.
Cookies and Embedded Content
This website does not use advertising or analytics trackers.
